12" black vinyl record, 33 RPM stereo LP with tan CBS Masterworks center labels and catalog number MS 7506 (matrix numbers XSM 139437 / XSM 139438), housed in its original white Columbia Masterworks cardboard sleeve still in shrinkwrap, featuring a stylized multi-hand portrait illustration of Chopin on the front and track listing and liner notes on the back.
Back-cover notes outline Chopin's life and compositional style and provide information on the participating ensembles, including the Philadelphia Orchestra, New York Philharmonic, Columbia Symphony Orchestra, conductors Eugene Ormandy and Andre Kostelanetz, and pianist Philippe Entremont.
